CareSource MyKids Program Enrollment

Thank you for your interest in the CareSource MyKids Program!

Please fill out this form to sign up. Once you’re done, we will check to see if you qualify to join. Each child must be signed up one at a time. You have to be a CareSource member to join the CareSource MyKids Program.

This number is found on the front of your Member ID Card.

Once enrolled and after completing the first rewardable activity, your child's reward card will be mailed.
