Request a Printed Provider Directory

We want you to have the most accurate information when choosing your health care providers. There are a couple of ways to find a provider.

The easiest way to find a provider is using our online search tool, Find a Doctor. Find a Doctor is updated daily. This means it always has the most current information. Using the tool, you can see which providers are accepting new patients or are close to where you live. You’re also able to filter providers by specialty, language, gender, and more.

You can also request a printed directory. Downloadable PDF directories by county here. Since provider information can change daily, a printed copy may not be accurate by the time you get it in the mail. This is why most members use Find a Doctor.
